Your eyecare provider has referred you into the eyecbetter glaucoma management program (GMP). The eyecbetter GMP is specifically designed to manage your glaucoma treatment journey in your own daily environment, so you have support outside of the short time you spend in your provider’s office. Our tools, support, and guidance help you understand and manage your glaucoma while adopting eye drop medication adherence and lifestyle habits to help you maintain a lifetime of vision.
The eyecbetter program is fueled by seewell™ our proprietary vision and life assessment that focuses on your activities of daily living, physical independence, social wellbeing, and medication tracking.
You are accessing the program through the eyecbetter app on your phone. Your responses and scores are reviewed, assessed and reports are sent to your ordering provider.
Personalized daily insights to help you maintain your vision and overall wellbeing. Our health tips and advice are always available. There is also education about glaucoma with questions for self-assessment of your understanding. Education was created from decades of questions from patients and can help you learn from what others have experienced.
Well-timed reminders can make a big difference. We will guide you along your eye drop journey and provide coaching to ensure that you take your medication on time and that the drops are administered properly.
Our clinical care coordinators can help you at just the right time by sharing vision health resources, tips, and coaching that is personalized for you.
Eyecbetter is coordinating care under the direction of your provider, we are an extension of your provider’s office/clinic. Your enrollment was initiated by your ordering provider, and they are overseeing your plan of care. Eyecbetter does not provide medical advice. The role of eyecbetter is limited to supporting, coordinating, and facilitating your engagement with the products and services via our app and program coordinators. The care management services provided by eyecbetter work in conjunction with your provider and allow for continuous care over the calendar month; you are enrolled in a six-month program.
If at any time you wish to disenroll from the program, please contact us at gmp@eyecbetter.com or let your eye care provider know. You can also talk to your personal care coordinator.
Note: Full termination of services will be at the end of the month in which you are currently enrolled.
